Richard Clarke Agnew, M.D., FACS

Dr. Agnew is currently serving as Chief, Department of Cardiovascular Surgery at St. Luke's Hospital. He joined Cardiothoracic & Vascular Surgical Associates in 1984. After graduating from Emory University in Atlanta with a B.S. Degree in Chemistry, Dr. Agnew attended Vanderbilt University Medical School and received his M.D. in 1975. He continued his residencies in general surgery and thoracic surgery at the Vanderbilt University Hospital.

Dr. Agnew is board certified by the American Board of Surgery and the American Board of Thoracic Surgery. He also is certified by the National Board of Medical Examiners.

After his residency, Dr. Agnew went west to Seattle, Washington where he was on the staff of the Mason Clinic and the Virginia Mason Hospital.

Joining Cardiothoracic & Vascular Surgical Associates in 1984, Agnew has medical appointments to St. Vincent's Medical Center, St. Luke's Hospital, Memorial Medical Center and Orange Park Medical Center.

Michael K. Bluett, M.D., FACS

Dr. Bluett currently serves as Chief, Department of Cardiovascular and Thoracic Surgery at St. Vincent's Medical Center. He joined Cardiothoracic & Vascular Surgical Associates in 1988. After receiving a Bachelor of Science degree in Biology with highest honors from Eckerd College, Dr. Bluett graduated from the University of Florida Medical School. He served his residency in General Surgery at Vanderbilt University and became Chief Resident in General Surgery at Vanderbilt University in 1985. He served a residency in thoracic surgery at the University of Michigan prior to returning to Florida and joining FT&CA.

Dr. Bluett also served as Chief, Cardiovascular and Thoracic Surgery at the Orange Park Medical Center from 1989-1994. He also served as Chief of Thoracic Surgery at Riverside Hospital. He is board certified by both the American Board of Surgery and the American Board of Thoracic Surgery. He maintains membership in the American Medical Association, the Society of Thoracic Surgeons and the Duval County Medical Society. Bluett also is a Council Member of the Florida Thoracic and Cardiovascular Surgeons.

Dr. Bluett has written and presented numerous papers on cardiovascular and thoracic surgery subjects. He is a regular participant in the annual programs of the Heart and Lung Institute, sponsored by St. Vincent's, Inc. with a program entitled Cardiopulmonary Update.

Charles Donaldson Cousar, M.D., FACS

Dr. Cousar is a native of Jacksonville and has been a member of Cardiothoracic & Vascular Surgical Associates Associates since 1990. He is a second generation thoracic surgeon serving the needs of North Florida patients. After receiving an AB degree in history from Princeton University, he  graduated from the Johns Hopkins University School of Medicine. He served his residency in both General Surgery and Cardiothoracic Surgery at Johns Hopkins Hospital in Baltimore.

Dr. Cousar is board certified by both the American Board of Surgery and the American Board of Thoracic Surgery. He maintains membership in the American College of Surgeons and the Society of Thoracic Surgeons.

John Kenneth Koster, M.D., FACS

Dr. Koster, currently the Medical Director of the Vascular Laboratory at Baptist Medical Center, joined Cardiothoracic & Vascular Surgical Associates in 1992. He attended the College of Medicine at the University of Kentucky where he was the valedictorian of his graduating class. He served his residencies in both general surgery and cardiothoracic surgery at the Kentucky Medical Center in Lexington, Kentucky.

Dr. Koster then was appointed Clinical Fellow and Chief Resident- -Thoracic Surgery at Peter Bent Brigham Hospital-Harvard Medical School, in Boston, Massachusetts. He was also a Research Fellow in Surgery at the Harvard Medical School.

After completing his training, Dr. Koster served on the faculty of the Harvard Medical School successively as a Tutor in Medical Sciences, Instructor in Surgery, and then Assistant Professor of Surgery. He came to Jacksonville in 1981 to serve as Chief of Thoracic and Cardiovascular Surgery at Baptist Medical Center.

During his career, he has published numerous articles pertaining to cardiothoracic and vascular procedures and case management in journals such as the Journal of Surgery, the Annals of Thoracic Surgery and the Journal of Thoracic & Cardiovascular Surgery. Dr. Koster is board certified by both the American Board of Surgery and the American Board of Thoracic Surgery.

Raymond Lee, M.D., FACS

Dr. Lee joined Cardiothoracic and Vascular Surgical Associates in 1998. He attended the Massachusetts Institute of Technology in Cambridge, Massachusetts for his undergraduate career where he studied biology and chemical engineering, He then attended Harvard Medical School in Boston, Massachusetts where he graduated with Magna Cum Laude honors.

Dr. Lee served his General Surgery internship and residency at the Massachusetts General Hospital in Boston. During that time, he worked during a research fellowship to develop and evaluate a hemoglobin blood substitute which is currently in FDA clinical trials. Dr. Lee then completed his cardiovascular and thoracic surgery training at the Massachusetts General Hospital.

Dr. Lee is board certified by both the American Board of Surgery as well as the American Board of Thoracic Surgery. Dr. Lee has medical appointments at St. Vincent's Medical Center, Baptist Medical Center (downtown and Beaches campuses), Memorial Medical Center, and St. Luke's Hospital.

Mark Alexander Mostovych, M.D. FACS

Dr. Mostovych currently serves as Chief, Department of Cardiovascular & Thoracic Surgery at St. Vincent's Medical Center. He joined Cardiothoracic & Vascular Surgical Associates in 1996. Prior to this he practiced cardiovascular and thoracic surgery including heart transplantation and pediatric congenital heart surgery at Baylor Medical Center and Methodist Medical Center in Dallas, Texas.

After receiving a Bachelor of Science Degree in Biology from the University of Kentucky, Dr. Mostovych graduated from the University of Louisville School of Medicine with Alpha Omega Alpha honors. He served his general and cardiothoracic surgical Chief residencies at Harvard's Massachusetts General Hospital. He also served as Chief Resident at Boston Children's Hospital in pediatric congenital heart surgery.

Dr. Mostovych is board certified by the American Board of Surgery and the American Board of Thoracic Surgery. He is also a fellow of the American Board of Surgery and a fellow in the American College of Chest Physicians. He maintains memberships in the Society of Thoracic Surgeons, the Massachusetts General Hospital Surgical Alumni Society, the Aldo Castaneda Pediatric Heart Surgery Society, and the Duval County Medical Society.

Dr. Mostovych and his wife Rhonda have three children; Alexander, Sophia, and Stephan. Dr. Mostovych is a dedicated physician, husband, and father.

Derek David Muehrcke, M.D., FACS

Dr. Muehrcke joined Cardiothoracic & Vascular Surgical Associates in 1996. Prior to this he practiced as a staff surgeon at the Cleveland Clinic, and was involved in numerous research and clinical protocols involving adult cardiac surgery.

Dr. Muehrcke received his Bachelor of Arts degree and Medical Doctor degree in a combined 7year program at Grinnell College in Iowa and Rush Medical College in Chicago.

As an undergraduate at Grinnell, Dr. Muehrcke was elected an academic, allAmerican football player and earned Phi Beta Kapa honors. After matriculating on to Rush Medical College in Chicago, he earned Alpha Omega Alpha honors. He received his general and adult cardiothoracic surgical training at Harvard Medical School, Massachusetts General Hospital. He obtained additional training as Chief Resident at Boston Children's Hospital in pediatric congenital heart surgery.

Dr. Muehrcke is board certified by the American Board of Surgery and the American Board of Thoracic Surgery. He has authored over forty original articles on basic science as well as clinical research, and has lectured extensively nationally and internationally. He routinely reviews articles for publication for the Annals of Thoracic Surgery, and maintains memberships in the Society of Thoracic Surgeons, Massachusetts General Hospital, Surgical Association, and Aldo Casteneda Pediatric Surgical Society, as well as the Duval County Medical Society

Dr. Muehrcke and his wife have one child, David.

Robert J. Still, M.D., FACS

Dr. Still has been a practicing Cardiovascular and Thoracic surgeon in Jacksonville, Florida since 1991. During this time he has served as Chief of Cardiovascular and Thoracic Surgery at Baptist Hospital, and Chief of Thoracic and Vascular Surgery at Methodist Hospital.

Dr. Still graduated Summa Cum Laude from the University of Virginia and AOA from the Johns Hopkins School of Medicine. He then served his general surgery residency at Massachusetts General Hospital in Boston, and completed his thoracic and vascular surgical residency at the Texas Heart Institute with Dr. Denton Cooley.

Dr. Still is Board Certified by the American Board of Surgery, The American Board of Thoracic Surgery, and holds a Certificate of Special Training in vascular surgery. He maintains membership in the Society of Thoracic Surgery, Massachusetts General Hospital Surgical Alumni Society, and the Denton A. Cooley, M.D. Society.

Dr. Still and his wife, DeAnna, are the parents of triplets, Robert, Jake, and Caroline.

J. Theodore Wingard, M.D., FASC

Dr. Wingard joined Cardiothoracic & Vascular Surgical Associates in 1978 and is the senior member of the group. He attended the University of South Carolina on a football scholarship where he received his Bachelor of Science degree and graduated from the Medical University of South Carolina.

Dr. Wingard served his surgical internship and surgical residency at Johns Hopkins Hospital in Baltimore. During this time, he was a visiting surgical resident at the Reza Pahlavia Hospital in Tehran, Iran; he worked with Dr. William McGowan in Dublin, Ireland and he was a cardiac fellow at Emory University Hospital in Atlanta. Georgia.

He is board certified by the American Board of Surgery and the American Board of Thoracic Surgery; and has medical appointments at Baptist Medical Center, Columbia Memorial Hospital, St. Luke's Hospital, St. Vincent's Medical Center and Columbia Orange Park Medical Center.

Alfred D. Harding, Jr., M.D., FACS

Dr. Harding has practiced vascular surgery in Jacksonville since 1990. He is a Missouri native and obtained a B.S. degree in Chemistry with honors from the University of Missouri. He then spent a number of years at the University of MissouriColumbia School of Medicine where he received his M.D. in 1983 and completed his residency in Surgery. Following residency he remained on the teaching faculty at the University of Missouri as Assistant Professor of Clinical Surgery. He then completed his fellowship in Vascular surgery before moving to Jacksonville.

Dr Harding is certified by the American Board of Surgery in General Surgery and Vascular Surgery.

Dr. Harding has a particular interest in noninvasive vascular laboratory studies in the diagnosis and treatment of vascular disorders. He is the medical director of the ICAVL certified vascular lab operated by the group
